We went through a broker to look at all options.

They set things up based on being full timers.

The one we used is based out of Ohio but does other states as well. If you like I can provide contact information.

In general a broker should be able to help you setup the best for what you need.
 
We went full time prior to Medicare. One of the reasons we picked Florida is because they were (at the time) the only of the 3 popular full time states ( FL, TX and SD) offering PPO nationwide coverage. In our case, my DW was a Hospital Nurse (good plan) and we extended her plan which was available in FL.
Per above, locate a broker in the state you choose to domicile in....
 
I set up my domicile in Florida using St. Brendan's Isle Mail Forwarding Service in order to qualify for individual PPO Health Insurance. I used Florida Blue, a subsidiary of Blue Cross Blue Shield. I don't know of a broker, but I agree with @JRS and recommend one to shop the plans for you once you know where you will be domiciled.
